Currently serving Canterbury families, Senior Transitions provides comprehensive support for elderly individuals and their families during the downsize from independent living to retirement or residential care facilities / Rest Homes. Services include medical advocacy, Rest Home search and booking, assistance with documentation, moving logistics, and asset management. Senior Transitions also offer Deceased Estate Clearance. The goal is to reduce the emotional, logistical, and time burden on both the elderly and their families.
How we can partner with your family
Below is a list of just some of the services we can provide to guide, assist and advocate for you. All with the goal of reducing the time and stress involved for you. We also understand the financial burden these times can bring, and we may have options available to remove any upfront costs for our services.
Chaperone – Your family’s eye’s and ears
We can attend medical appointments to advocate for outcomes, ensure clarity of communication and sharing of the right information.
Deceased Estate Clearance
If a loved one has passed, and there is no local family or support, Senior Transitions can take care of the estate, from actioning the Will, to selling, donating, disposing of property, arranging the cleaning, gardening and maintenance of the home and getting it ready for sale. We can also suggest local Real Estate Agents.
Finding the next home
We can find, inspect, and make recommendations on available rest homes and retirement facilities, as well as arrange visits for you (including transport if that helps).
We can then assist with booking and admission arrangements.
Documentation
We understand that all of the admission, Residential Care Subsidy and legal paperwork can be daunting, so we can walk you through it step by step.
Moving
We will gladly make all of the arrangements to relocate you into your new home, ensuring that it is ready and waiting for you to arrive.
Your property
We can arrange for your belongings to either move with you, go into storage for later, or be donated where possible. We can get your home ready for the next steps, and we can then make arrangements for your real estate needs if your property is to be sold.
If an unexpected event means you need urgent support – give us a call.
Sometimes there isn’t much time to prepare, especially if support is a long way away.
If a medical event or an accident means someone you care about needs a chaperone, or to find a Rest Home or Residential Care facility before family support can assist, give us a call and we can be there.
